Job summary

Residential Home Health and Residential Hospice is hiring a Speech Language Pathologist (SLP) for home health care in the Glen Ellyn area. The role offers a $15,000 total bonus potential, including a $10,000 first-year success bonus and a $5,000 sign-on bonus paid within 18 months.

You will deliver skilled care in patients’ homes, perform evaluations, educate caregivers, and collaborate with an interdisciplinary team, traveling within the assigned territory. An equal opportunity employer.

Qualifications

• Active Speech Language Pathologist with state license.
• Minimum of 1 year experience as an SLP.
• Valid driver’s license and the ability to drive ~20% of workday to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in home health.
• Perform comprehensive evaluations of speech, language, cognitive-communication, and swallowing; develop treatment plans.
• Educate and empower patients and caregivers on exercises.
• Ensure compliant EMR documentation and think critically to solve problems.
• Work independently but collaborate with an interdisciplinary team.
• Travel to patient homes within assigned territory.

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life
• Educate and empower patients and caregivers on therapeutic exercises
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

SLP Qualifications

• Active Speech Language Pathologist, SLP license in the state you work
• One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
