Job summary

CenterWell Home Health seeks a licensed Speech-Language Pathologist to provide in-home services, assess and treat speech, language and swallowing disorders, and coordinate care with the clinical team.

Responsibilities include planning care, documenting progress, educating patients and families, and maintaining confidentiality. Requires CPR and reliable transportation; driving is essential.

Qualifications

• Current and unrestricted license as a Speech-Language Pathologist.
• CPR certification required and valid driver's license with reliable transportation.

Responsibilities

• Evaluate, direct and provide speech-language pathology service to patients in home settings.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe and report changes in patient condition to supervisor or physician.
