Job summary

Enhabit Home Health & Hospice in the United States is seeking a Nurse Scheduling Coordinator to ensure seamless assignment of patients to field clinicians and maintain care continuity across visits.

The role emphasizes proactive communication with field staff, patients, physicians, referral sources, and caregivers, plus managing on-call notebooks and hospitalization logs to optimize care coordination and patient outcomes.

Qualifications

• Must have high school diploma or equivalent.
• Licensed practical or vocational nurse with valid state license.
• Minimum one year of clinical experience in healthcare.
• CPR certification required.
• One year of home health, hospice, or pediatric experience within last 24 months.
• Valid state driver\'s license and automobile liability insurance.
• Basic technology skills including operation of a mobile device.
• Preferred bilingual English/Spanish.

Responsibilities

• Schedule patients to field clinicians to ensure care continuity.
• Communicate with field staff, patients, physicians, referral sources, caregivers and other service providers.
• Manage the on‑call notebook and hospitalization logs to enhance communication among stakeholders.
