Four Seasons Certified Home Health Agency is seeking a Scheduling Coordinator to manage CHHA staff schedules and assignments, ensuring timely home care for patients. You will coordinate visits, handle conflicts, and communicate with clients and caregivers to maintain smooth operations.

The role requires strong communication skills, the ability to multitask, and familiarity with Netsmart/HHA Exchange. In-person work with competitive benefits is offered.

Responsibilities

• Plan and coordinate CHHA visits, ensuring adequate coverage and timely care.
• Address scheduling conflicts, manage no-shows, and resolve staffing issues.
• Communicate with patients, families, caregivers, and team members regarding schedules.
• Maintain records of patient visits, care plans, and scheduling adjustments.
• Collaborate with nurses and case managers to ensure coordinated care.
