Job summary

Cantex Continuing Care Network in Fort Worth, TX is seeking a detail-oriented Scheduler to coordinate patient visits and clinician schedules across SN, PT, OT, ST, MSW, and HHA. You will ensure efficient staffing, continuity of care, and smooth workflow between clinical teams and patients.

Responsibilities include maintaining accurate visit schedules, communicating changes to clinicians and patients, providing coverage for PTO and call-outs, and generating utilization reports to drive

Qualifications

• High School diploma or equivalent; college coursework preferred.
• LVN preferred, but not required.
• Experience scheduling across SN, PT, OT, ST, MSW, HHA in home health.
• Strong knowledge of medical terminology and home health operations.
• Proficient in Microsoft Excel and Outlook; reporting experience helpful.
• Ability to interpret productivity and utilization reports.

Responsibilities

• Maintain accurate scheduling of patient visits and clinician assignments.
• Coordinate across multiple disciplines to ensure visit completion.
• Communicate schedule changes to clinicians and patients; arrange coverage.
• Provide staffing support for PTO, call-outs, and extended leave.
• Monitor visit utilization, missed visits, and clinician productivity.
• Generate reports to support operational decisions and ensure Plan of Care alignment.
• Serve as central coordination point between clinical staff, patients, and leadership.
• Maintain compliance with agency policies and scheduling standards.
• Support day-to-day operations with strong time management and organization.
