Job summary

TheraCare Home Health in Houston, TX is seeking a highly organized Scheduler to support daily operations in a fast-paced home health environment. You will coordinate patient visits, manage clinician schedules across multiple disciplines, and help ensure continuity of care for patients and efficient use of staff resources.

Ideal candidates will have a High School diploma (LVN preferred), experience with HomeCare HomeBase (HCHB) or similar systems, and proficiency in MS Office.

Qualifications

• High School diploma required; college coursework preferred.
• LVN preferred, but not required.
• Experience scheduling across multiple disciplines (SN, PT, OT, ST, MSW, HHA) preferred.
• Proficiency in Microsoft Office (Excel, Outlook).
• Ability to interpret productivity and utilization reports.

Responsibilities

• Maintain accurate scheduling of patient visits and clinician assignments.
• Coordinate schedules across multiple disciplines to ensure continuity of care.
• Communicate schedule changes to clinicians and patients.
• Provide staffing support for call-outs, PTO, and extended leaves.
• Monitor visit utilization, missed visits, and clinician productivity.
• Generate reports to support operational decisions.

In this role, you will coordinate patient visits, manage clinician schedules across multiple disciplines, and ensure efficient utilization of staff and resources.
