Enhabit Home Health & Hospice is hiring a Scheduling Coordinator to manage patient assignments to field clinicians in Ada, OK. You will collaborate with clinicians, physicians, referral sources, and caregivers to ensure seamless care coordination and timely follow-ups.

The role requires at least 1 year of clinical healthcare experience, an active LPN license, CPR, and a valid Oklahoma driver’s license. In-office attendance is essential to support supervision and teamwork.

Qualifications

• Must have a High School Diploma or equivalent.
• Active LPN license and CPR certification are required.
• Minimum one year of clinical healthcare experience.
• Must have and maintain a valid state driver license and auto liability insurance.

Responsabilités

• Schedule patients to field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, referral sources, and caregivers to maintain care continuity.
• Manage on-call notebook and hospitalization logs to improve communication among stakeholders.
