Job summary

WelbeHealth in Long Beach, CA is seeking a Home Care Scheduler to coordinate scheduling and tracking of Home Health and Home Care Assistant assignments, ensuring high-quality care for participants. This role reports to the Home Care Supervisor and emphasizes outstanding customer service and precise record-keeping.

You will maintain EMR/EHR records, communicate with participants, families, vendors and staff, and implement procedures to improve the scheduling process.

Qualifications

• High school diploma or equivalent.
• Minimum of one (1) year of experience as a scheduler in a home health or home care agency.
• Minimum of one (1) year of experience working with an EMR and scheduling system preferred.

Responsibilities

• Coordinate scheduling of Home Care Assistant/Personal Care Assistant assignments as directed by Home Health RNs and Home Care Supervisor.
• Complete required authorizations in EMR for approved services.
• Ensure all medical records for clinicians, home care aides, vendors, and visits are uploaded into the EHR.
• Maintain current records of all scheduled home health appointments and assignments.
