Job summary

Qualifications

• Healthcare licensure or equivalent required.
• Minimum three years of case management or nursing in a home setting for frail/elderly individuals.
• CPR and First Aid certification within 180 days of hire preferred.

Responsibilities

• Review and audit Participant Care Plans and coach on documentation for compliance.
• Oversee pre-enrollment assessments and collaborate with MOE team.
• Manage direct reports including hiring, training, supervising and mentoring.
• Lead internal investigations and coordinate with HR for conflict resolution.
• Lead daily HCC meetings and conduct IDT case review coaching.
