Job summary

WelbeHealth seeks a Home Care Supervisor to oversee in-home care teams, coordinating scheduling for Home Care Assistants and managing relationships with external vendors. The role ensures the highest level of participant care within a value-based model.

You will monitor compliance, provide coaching, and collaborate with the interdisciplinary team to support home exercise programs and care plans for seniors in their homes.

Qualifications

• Oversee a team of Home Care Assistants delivering personal care to frail or elderly participants.
• Supervise and mentor staff, ensuring training and credentials meet WelbeHealth standards.
• Ensure compliance and accurate documentation across the care team.

Responsibilities

• Oversee Home Care Assistant team and scheduling for in-home care services.
• Train and provide feedback to Home Care Assistants; ensure certifications are current.
