Job summary

WelbeHealth is seeking a Home Care Supervisor to oversee the delivery of personal care services to participants in their homes, coordinating scheduling for Home Care Assistants and managing relationships with external vendors.

You will work with the Home Health Manager and Rehabilitation teams to ensure care plans are executed and staff are properly trained. A strong background in supervision and compliance is required.

Qualifications

• High school diploma or equivalent; bachelor’s degree preferred.
• Minimum 1 year of supervisory experience overseeing Home Care Assistants.
• 2 years of supervisory experience with mentoring ability.
• Reliable transportation required.
• CPR and First Aid certifications within 180 days of hire (preferred at hire).
• Experience supporting home care with training on care plans is a plus.

Responsibilities

• Oversee the Home Care Assistant team delivering activities of daily living in participants’ homes.
• Interview, hire, onboard, train and provide feedback to Home Care Assistants.
• Provide feedback to agencies/contracted vendors on performance and interactions.
• Maintain records of certification for Home Care Assistants and ensure documentation is current.
• Audit Home Care Assistant documentation for accuracy and timeliness; provide training as needed.
• Collaborate with Home Health Manager and Rehabilitation to ensure staff training for home exercise programs.
