Job summary

WelbeHealth is seeking a Home Care Coordinator in Rosemead, CA to conduct in-home care assessments and coordinate personal care delivery. Reporting to the Home Care Manager, you will liaise with participants, physicians, nursing and therapy teams to arrange schedules and services.

The role requires at least three years in case management or nursing with frail elderly populations; a bachelor’s degree is preferred.

Qualifications

• Healthcare/medical licensure or equivalent; at least 3 years of professional experience.
• Bachelor’s degree preferred.
• Minimum of 3 years of case management or nursing experience in a clinical/home setting with frail elderly population.
• Nursing knowledge and training to treat frail, elderly participants.

Responsibilities

• Coordinate incoming calls related to participants, physicians, and agency services regarding physician orders, participant questions, and referrals.
• Communicate with participants by phone and coordinate effectively with nursing, therapy, aides, social services, and physicians regarding schedule changes, test results, and other related updates.
• Collaborate with Home Care Services staff to track and monitor home care delivery and scheduling hours.
• Partner with the Marketing team to support the enrollment of prospective participants into the program.
• Assist with staffing and scheduling activities, soliciting, and manager input.
• Participate in end-of-life care, coordination, and support.
