Job summary
Angels Care Home Health is seeking a Provider Support Registered Nurse (RN) Case Manager to coordinate all aspects of patient home care. You will interact with multiple medical disciplines, perform admission visits to assess eligibility, and develop comprehensive plans of care.
The role requires travel to provider branches, with occasional overnight stays. The ideal candidate holds an RN license, has 1 year of RN experience (2 years preferred), and is willing to travel within the provider
Qualifications
- Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
- Current state RN license.
- CPR certification and Hepatitis consent/declination.
- Willingness to travel to all provider branches.
- Valid driver’s license and reliable transportation.
Responsibilities
- Coordinate all aspects of the patient’s home care.
- Interact with other medical disciplines as needed and provide professional care.
- Perform admission visits and determine eligibility for home care services.
- Develop plan of care to be followed and monitor progress.
- Travel to branches and participate in occasional overnight stays.
