Job summary
Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate comprehensive home care for patients in the community. You will perform admission visits, assess eligibility for home care services, and develop individualized care plans in collaboration with the patient and their family.
The role requires an active RN license, a valid state driver’s license, CPR certification, and reliable transportation.
Qualifications
- Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
- Current state license as a Registered Nurse.
- Current state Driver’s License.
- One-year experience as a Registered Nurse, two preferred.
- Proof of current CPR and Hepatitis consent/declination.
- Reliable transportation with valid and current auto liability insurance.
Responsibilities
- Coordinate all aspects of the patient’s home care.
- Interact with other medical disciplines as needed and provide professional care.
- Perform admission visit to the patient in the home and determine eligibility for home care services.
- Develop the plan of care to be followed.
