Angels Care Home Health is seeking a Provider Support Registered Nurse (RN) Case Manager to coordinate all aspects of patient home care. You will perform admission visits, assess eligibility for home care services, and develop the plan of care in collaboration with medical teams.

This full-time role requires an active RN license, at least 1 year of RN experience, CPR, and a willingness to travel to branches within the provider. Occasional overnight stays may be required.

Qualificações

• Graduate of an accredited nursing program.
• Current state RN license.
• CPR certification and Hepatitis consent/declination.
• Willingness to travel to multiple branches within the provider.

Responsabilidades

• Coordinate all aspects of the patient’s home care.
• Perform admission visits and determine eligibility for home care services.
• Develop and monitor the plan of care with the medical team.
• Collaborate with other medical disciplines as needed.
