Job summary

Angels Care Home Health seeks a Provider Support Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care and to interact with other medical disciplines as needed.

The nurse will perform admission visits, assess eligibility for home care services, and develop the plan of care to be followed. This is a full-time role serving designated areas with occasional overnight stays and travel between branches.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse (RN).
• One-year experience as a Registered Nurse (RN), two preferred.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed.
• Perform the admission visit to the patient in the home and determine eligibility for home care services.
• Develop the plan of care to be followed.
