Angels Care Home Health seeks a Provider Support Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will interact with other medical disciplines as needed and perform the admission visit to the patient in the home to determine eligibility and develop the plan of care.

This full-time role covers a designated area within a provider where there is vacancy or higher census, and requires travel to branches.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse (RN).
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Must be willing to travel to all branches within a provider.
• Candidate must be prepared for occasional overnight stays.
• Current state Driver’s License.
• Reliable transportation with valid and current auto liability insurance.

Responsabilités

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed & provide professional care to the patient.
• Perform the admission visit to the patient in the home & determine the patient’s eligibility for home care services & develop the plan of care to be followed.
