Angels Care Home Health is seeking a Provider Support Registered Nurse (RN) Case Manager to coordinate home care for patients. The role includes admission visits, eligibility determination, and developing the plan of care across multiple medical disciplines.

The position is full-time in a designated area with travel to branches, occasional overnights, and opportunities for career advancement, benefits, and ongoing training.

Qualifications

• Graduate of an accredited nursing program (diploma/ASN/BSN).
• Current state RN license.
• Valid driver's license and reliable transportation.
• CPR certification and hepatitis consent/declination.
• 1 year RN experience; 2 years preferred.
• Willing to travel to branches and occasional overnights.

Responsabilités

• Coordinate patient home care across disciplines.
• Perform admission visit and determine home care eligibility.
• Develop and follow the plan of care.
• Interact with branches; travel as needed.
• Ensure quality patient outcomes.
