Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will interact with medical disciplines, provide professional in-home care, and perform admission visits to determine eligibility and develop the plan of care to be followed.

Requirements include an active RN license, driver’s license, CPR, and reliable transportation; one year of RN experience (two preferred).

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of patient home care.
• Interact with other medical disciplines as needed and provide professional care to the patient.
• Perform admission visit in the home and determine eligibility for home care services.
• Develop and follow the plan of care for the patient.
