Job summary

Tufts Medicine Care at Home is seeking a Home Health Registered Nurse to deliver comprehensive in-home nursing care using the nursing process. You will assess, intervene, and support patients and families, coordinating with care teams to avoid unnecessary hospitalizations.

Qualifications

• Active RN license in the state where services are provided.
• Minimum 1 year of acute medical/surgical nursing experience.
• Basic Life Support (BLS) certification required.
• CPR certification by hire date deadline if applicable.

Responsibilities

• Assess patients’ physical and psychosocial needs and provide ongoing interdisciplinary assessment.
• Coordinate care plans with patient/family and health care team.
• Document patient data and care activities in the electronic record promptly.
• Complete orders, levels of care, and OASIS per policy; reconcile medications.
• Provide nursing coverage evenings, nights, weekends, and holidays as needed.
• Ensure safety, emergency response, and adherence to standards of nursing practice.
