Job summary

Tufts Medicine Care at Home is seeking a Home Health Registered Nurse in Lowell, MA and surrounding areas. You will manage comprehensive home health services, including assessments, interventions, and patient advocacy, working with a multidisciplinary team to avoid unnecessary hospitalizations.

The role requires an active RN license in the service state, a valid driver’s license, and at least one year of acute medical/surgical nursing experience.

Qualifications

• Active RN license in the state of service; if not licensed, obtain within 60 days.
• Valid state issued Driver’s License.
• One year of acute medical/surgical nursing experience.
• Basic Life Support (BLS) certification.
• If hired before 9/28/2026, CPR required at hire; after 9/28/2026, CPR/BLS within 30 days.

Responsibilities

• Assess patients’ physical and psychosocial needs following standards of practice.
• Develop and implement plans of care with the health care team.
• Coordinate care and discharge planning with team members.
• Document patient data and communications in the medical record.
