Tufts Medicine Care at Home in Lowell, MA is seeking an experienced Home Care Nurse (RN) to provide professional nursing care in patients' homes under physician orders. You will triage calls, coordinate care with clinicians, and support the management team to ensure timely, quality services.

Requires an RN license and 3 years of community health/home health experience; telephone triage experience is preferred. This is a full-time role with competitive pay and benefits.

Qualifications

• Registered Nurse (RN) license required.
• Three years of community health or a combination of acute medical/surgical and home health experience required.
• Telephone triage experience preferred.

Responsibilities

• Coordinate clinical services consistent with agency objectives.
• Provide direct patient care and coordinate care with families.
• Triages physician calls and coordinates actions with field staff.
• Document concisely to support reimbursement and audits.
• Assist with orientation and preceptor of clinical staff.
• Collaborate with disciplines to ensure efficient operations.
