Adecco Healthcare & Life Sciences is seeking an experienced Registered Nurse (RN) for a Home Health contract in Holland, OH. This 13-week assignment provides skilled nursing care and case management to patients in their homes across the service area.

Location: Holland, OH. Schedule: Mon–Fri, 8:00 AM–4:30 PM (40 hours/week). On-call/weekend coverage required. Travel throughout Toledo, Fostoria, Findlay and nearby counties.

Qualifications

• Current Registered Nurse (RN) license in Ohio.
• Home health experience preferred.
• Experience completing OASIS assessments preferred.

Responsabilités

• Provide skilled nursing care and case management to patients in their homes.
• Complete SOC assessments and other required home health visits.
• Perform comprehensive patient assessments and develop individualized care plans.
• Complete OASIS assessments and maintain accurate clinical documentation.
• Monitor patient progress and report changes in condition to the healthcare team.
• Coordinate care with physicians, patients, families, and other healthcare professionals.
• Travel between patient visits within the assigned coverage area.
• Participate in required on-call and weekend coverage.

Adecco Healthcare & Life Sciences is seeking an experienced Registered Nurse (RN) for a Home Health contract in Holland, OH. This is a 13-week assignment providing skilled nursing care and case management to patients in their homes throughout the assigned service area.

Job Details

Location: Holland, OH

Pay: $53 to $55.90/hr, mileage reimbursed between patient visits

Contract: November 2026 – March 2027

Schedule: Monday–Friday, 8:00 AM–4:30 PM (40 hours/week)

On-Call/Weekend: One week and one weekend required

Coverage Area: Toledo, Fostoria, Findlay, and surrounding counties

Responsibilities

Provide skilled nursing care and case management to patients in their homes

Complete Start of Care (SOC) assessments and other required home health visits

Perform comprehensive patient assessments and develop individualized care plans

Complete OASIS assessments and maintain accurate clinical documentation

Monitor patient progress and report changes in condition to the appropriate healthcare team

Coordinate care with physicians, patients, families, and other healthcare professionals

Travel between patient visits throughout the assigned coverage area

Participate in required on-call and weekend coverage

Requirements

Current Registered Nurse (RN) license in Ohio

Previous home health nursing experience

Experience completing OASIS assessments required

Homecare Homebase (HCHB) experience preferred

Ability to work independently in a field-based environment

Reliable transportation for travel between patient visits

Strong communication, organization, and documentation skills

Pay Details: $53.00 to $55.90 per hour
