Job summary

Apollo Home Healthcare is seeking a Registered Nurse (RN) to provide in-home nursing care in the Bay Area. The role offers full-time or part-time/per diem options and oversees a supporting LVN within a multidisciplinary team.

Qualifications

• Active California RN license.
• CPR/BLS certification.
• Home health experience preferred.
• Reliable transportation and ability to travel to patients’ homes.

Responsibilities

• Provide in-home nursing visits according to the physician’s plan of care.
• Assess patients’ health status and home environment.
• Develop and update individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on disease management and home care.
• Monitor patient progress and communicate changes to the physician and care team.
• Document visits and care in a timely manner.
• Coordinate care with therapists, aides, and other team members.
• Serve as case manager for assigned patients and support discharge planning.
