Job summary

Apollo Home Healthcare is seeking an RN to join our Home Health team in the Bay Area. You will provide in-home nursing care, assess patients, and develop care plans, working with physicians and therapists to coordinate services.

The role supports a Licensed Vocational Nurse, emphasizes independence in home settings, and offers full-time or part-time/per diem options with competitive benefits.

Qualifications

• Active California RN license.
• CPR/BLS certification.
• Strong communication and organizational skills.
• Reliable transportation and willingness to travel to patients' homes.
• Home health experience preferred.
• Experience with Kinnser/Wellsky preferred but not required.

Responsibilities

• Provide in-home nursing visits according to the physician's plan of care.
• Assess patients' health status including condition, history, and home environment.
• Develop and update individualized care plans based on patient needs.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on disease management and home care.
• Monitor patient progress and communicate changes to the physician and care team.
• Document visits and care in a timely manner.
• Coordinate care with therapists, aides, and other team members.
• Serve as case manager for assigned patients and support discharge planning.
