Job summary

Health at Home is seeking a dedicated RN to provide skilled nursing care in patients' homes. You will assess, plan, and coordinate home health services, while educating patients and families to improve health outcomes.

Key responsibilities include completing OASIS assessments, developing individualized care plans, and collaborating with an interdisciplinary team to ensure high-quality, compliant care in the home setting.

Qualifications

• Graduate of an accredited nursing program (ADN or BSN).
• Active RN license in the state of practice.

Responsibilities

• Deliver skilled nursing care including wound care, IV therapy, medication management.
• Perform OASIS assessments and data collection with accuracy.
• Develop individualized plans of care aligned with physician orders and patient goals.
• Educate patients and families on disease management and self-care.
• Coordinate with the care team to ensure cohesive, responsive services.
• Document accurately in the EMR and maintain timely records.
• Identify additional services and initiate referrals when appropriate.
• Provide supervision and guidance to Home Health Aides as needed.
• Participate in quality improvement initiatives and regulatory compliance.
