Job summary

Health at Home seeks a Registered Nurse (RN) to deliver compassionate in-home care, plan and direct home health services, and educate patients and families. You will assess, treat, and coordinate with the care team, while maintaining compliant documentation and upholding quality standards.

Responsibilities include completing OASIS assessments, designing individualized care plans, and providing wound care, IV therapy, and disease management.

Qualifications

• Graduate of an accredited nursing program with RN licensure in the practice state and any required by operations.
• Active CPR certification required; additional certifications welcomed.
• Minimum 1 year of clinical nursing in hospital, community health, or home health.
• Experience with OASIS and home health preferred; training provided.

Responsibilities

• Deliver skilled nursing care and perform treatments per care plan.
• Complete OASIS assessments and accurate documentation.
• Develop individualized plans of care with physician orders and goals.
• Educate patients and families on disease management and self-care.
• Coordinate with physicians and interdisciplinary team; act as Case Manager when needed.
• Maintain compliance with regulatory standards and quality initiatives.
