Job summary

UnitedHealth Group is seeking a Registered Nurse to provide patient care in home health settings as part of a collaborative interdisciplinary team. You will coordinate plans of care with physicians and ensure compliance with laws and agency policies, including OASIS and medication reconciliation.

This role involves supervising aides and LVNs, documenting care, and participating in staff orientation and quality initiatives.

Qualifications

• Must hold current unrestricted RN licensure in state of practice.
• Must have a valid driver’s license, vehicle insurance, and access to reliable transportation.

Responsibilities

• Develops, updates, and coordinates patient plans of care with physicians and interdisciplinary team.
• Supervises home health aides and LPN/LVN staff in accordance with regulations.
• Documents patient care accurately per policy and payer requirements; communicates with physicians, patients, families, and staff.
• Participates in orientation and precepting of new staff; contributes to performance improvement and patient safety initiatives.
• Demonstrates professionalism, punctuality, and adherence to agency standards; participates in on-call/weekend rotations.
• Function in diverse home care settings regardless of patient demographics.
