Job summary

Qualifications

• Active Florida State Professional License (RN)
• Minimum 1 year with Medicare Certified Home Health agency
• OASIS SOC Competency
• Knowledge of AHCA regulations/Medicare COPs
• Proficient with computers
• Flexible schedule
• Excellent oral and written communication
• Ability to read, write and understand English

Responsibilities

• Perform initial and ongoing home health assessments.
• Complete assessments including OASIS.
• Re-evaluate patient nursing needs and outcomes.
• Develop and revise the Plan of Care (POC).
• Initiate nursing procedures and discharge planning.
• Communicate with patients and families; participate in case conferences.
• Follow infection control protocols (OSHA).
• Coordinate services and inform physicians of changes.
• Supervise Home Health Aide and LPN as required.
• Complete timely documentation to ensure policy compliance.
• Maintain licenses and certifications.
• Float to other units as needed.
