Job summary

SA Nurses is rapidly growing and currently hiring Home Health Nurses to provide in-home nursing care after hospital discharge or post-acute settings.

Your role focuses on clinical care, patient education, and coordination with the care team to ensure high-quality outcomes for patients in the community.

Qualifications

• Clinical knowledge, interpersonal communication and compassionate care are essential.
• Time management skills are essential tools to possess.
• Experience in home health nursing is preferred.

Responsibilities

• Assess patients' condition during visits and chart observations.
• Perform evaluation tasks, including vital signs, medication review and wound care.
• Dress or redress wounds and assess healing progress.
• Collect data and prioritize problems and needs.
• Supervise CNA and LVN under assigned caseload.
• Document to company and compliance standards.
• Educate patients and families on home health care strategies.
• Coordinate with therapists and physicians involved in care plan.
• Make recommendations for devices or tools to improve quality of life.

Home Health Nurse Responsibilities

• Assess patients' condition during every visit and chart your observations
• Perform evaluation tasks, including vital signs and medication review, Oasis assessment, wound care and IV therapy.
• Dress or redress open wounds and assess progress of healing
• Collect data and prioritize problems and needs
• Supervise CNA and LVN under assigned to your caseload
• Document to company and compliance standards
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient's care plan
• Make recommendations for devices or tools that might improve the patient's quality of life
• Listen to the patient and respond to concerns or requests
