Job summary

UnitedHealth Group seeks a Registered Nurse for Home Health to provide and direct nursing care to patients in their homes, coordinating with physicians and the care team to ensure continuation of patient care. You will supervise aides, complete OASIS assessments, and maintain accurate documentation in line with agency policies.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Valid driver's license, vehicle insurance, and access to dependable transportation.
• CPR certification preferred or obtainable within 90 days of hire.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with physician and health team.
• Supervises home health aides and licensed staff per state/federal regs.
• Performs supervisory visits and ensures quality care delivery.
• Completes comprehensive assessments (OASIS) and medication reconciliation.
• Documents visits accurately and meets policy and payer requirements.
• Maintains timely communication with physicians, patients, families, and staff.
