Job summary

Health at Home is seeking a Registered Nurse to provide in-home skilled nursing care. You will assess, plan, and deliver care directly in patients’ homes, educating families and coordinating with the care team.

Responsibilities include OASIS assessments, care plan development, medication management, and ongoing education to empower patients to manage health at home. Valid RN license and CPR required.

Qualifications

• RN license in the state of practice (unencumbered).
• CPR certification through approved provider.

Responsibilities

• Perform OASIS assessments and data collection accurately.
• Develop individualized care plans aligned with physician orders and patient goals.
• Deliver skilled nursing interventions including wound care, meds, IV therapy.
• Educate patients and families on disease management and self-care.
