Job summary

Health at Home is seeking a compassionate Registered Nurse (RN) to deliver skilled home health services in patients’ homes. You will assess, plan, and implement care, coordinating with physicians and the interdisciplinary team.

You will educate patients and families, document accurately in EMR, and stay current with Medicare regulations and home health standards. Florida RN license and CPR are preferred.

Qualifications

• Graduate of an accredited nursing program
• Active, unencumbered RN license in state of practice
• Current CPR certification
• OASIS/home health experience preferred
• Experience in hospital/clinical home health settings beneficial

Responsibilities

• Deliver OASIS assessments and data collection with accuracy
• Develop individualized care plans with measurable goals
• Provide skilled nursing care including wound care, medications, IV therapy
• Educate patients and families on disease management and self-care
• Coordinate with physician and care team; act as Case Manager when assigned
• Document OASIS and visits accurately within required timeframes
• Identify opportunities for PT/OT/SLP/MSW/HHA referrals
• Participate in team meetings to improve patient outcomes
• Maintain current nursing knowledge per regs and standards
