Job summary

Elite Home Health seeks a dedicated Registered Nurse (RN) to provide and coordinate skilled nursing care for patients in their homes across Florida. The role emphasizes compassionate, patient-centered care and collaboration with an interdisciplinary team.

The RN will perform assessments, develop care plans, complete SOC and recertifications, and ensure compliance with Medicare and state regulations. A valid Florida RN license and CPR certification are required.

Qualifications

• Graduate of an accredited School of Nursing (ASN or BSN).
• Current Florida RN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform comprehensive and ongoing patient assessments.
• Complete Start of Care (SOC), recertification, reassessment, and other required visits.
• Complete OASIS assessments accurately and within required timeframes.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing services per physician orders and plan of care.
• Monitor patient condition and identify changes in health status.
• Communicate significant changes to physicians and care team.
• Coordinate care with physicians, therapists, LPNs, CNAs, and other team members.
• Provide direction and supervision to LPNs and CNAs as applicable.
• Document accurately in the EMR and ensure regulatory compliance.
• Participate in care coordination, team meetings, and required training.
