Job summary

Preferred Home Health Care & Nursing Services is seeking an experienced Home Health RN to provide high-quality nursing care in patients' homes across the region. You will conduct comprehensive assessments, develop individualized care plans, and administer treatments for complex, chronic, and post-acute needs.

You will educate patients and families on disease management, self-care, and home safety while collaborating with an interdisciplinary team to coordinate services and outcomes.

Qualifications

• Active RN licensure and ability to provide independent home-based nursing care.
• Strong clinical assessment, critical thinking, and care-plan management skills.
• Excellent communication and patient/family education abilities.
• Proficiency with electronic documentation and attention to detail.
• Ability to work effectively with interdisciplinary teams.
