Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of home-based care. You will conduct admission visits, determine eligibility for home health services, and develop comprehensive plans of care for patients in their homes.

Qualifications

• Graduate of an accredited nursing program.
• Current state RN license in Oklahoma.
• One year of RN experience; two preferred.
• Proof of CPR and Hepatitis vaccination or declination.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform admission visit to the patient in the home.
• Develop and implement the plan of care.
• Collaborate with other medical disciplines as needed.
