AccentCare, Inc. is seeking an RN Case Manager for Home Health in Humble, TX. The role involves managing patient care plans from admission through discharge, coordinating nurses and clinicians, and ensuring quality home health care.

The position is full-time and on-site with a typical Monday–Friday schedule. You will assess patient needs, update care plans, and communicate with physicians while documenting progress and ensuring timely care.

Formación

• RN license in state of operation
• Minimum 1 year RN experience
• Current CPR certification

Responsabilidades

• Assess nursing needs and manage the patient care plan.
• Complete initial admission assessment and patient history.
• Select appropriate nursing diagnoses.
• Review medications for therapy issues, allergies, and adverse reactions.
• Establish realistic, measurable goals aligned with diagnosis.
• Inform physician of assessment results and interventions.
• Implement nursing interventions within scope of practice.
• Re-evaluate patients' needs as required.
• Document assessments and discharge notes timely.
• Inform physician and staff of changes in patient condition.
