Job summary

AccentCare, Inc. is seeking an RN Case Manager for Home Health in Brenham, TX. This PRN role involves managing patient care plans from admission through discharge and coordinating with nurses and allied health staff to ensure quality care.

The ideal candidate has RN licensure in Texas, at least one year of RN experience, and CPR certification, with the ability to lead care team activities and document progress thoroughly.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as an RN in the state of operation.
• Minimum of one year of RN experience.
• Current CPR certification required.

Responsibilities

• Assess and manage patients' care plans from admission through discharge.
• Direct staff nurses, LPNs, and CHHAs in delivering the plan of care.
• Coordinate interdisciplinary needs and allied health clinicians.
• Document assessments, progress, and discharge notes in a timely fashion.
• Inform physicians of changes in patient condition and needs.
