The Christ Hospital Home Health Care is seeking a Registered Nurse Case Manager (RNCM) in the Cincinnati area. You will lead patient care from admission through plan of care completion, coordinate multi-disciplinary teams, and ensure documentation complies with Medicare and agency standards.

Ideal candidates hold an active RN license, have acute care experience, and can communicate effectively with patients, families, and physicians.

Qualifications

• Registered Nurse with valid state license and current credentials.
• Experience in acute care preferred; home health experience desirable.
• Ability to communicate clearly with patients, families and physicians.

Responsabilités

• Oversees overall patient care from start to end of episode.
• Updates doctor orders and notifies changes in patient condition.
• Manages multi-disciplinary care and coordinates field staff.
• Provides skilled nursing care at the patient’s home as ordered.
• Performs OASIS assessments to develop individualized care plans.
• Documents treatment, assessments and education per agency policies.
