Job summary

The Christ Home Health in Cincinnati, OH seeks an experienced Registered Nurse Case Manager (RNCM) to lead assessment, planning, and coordination of patient care in the home setting.

You will educate patients and families, ensure safety, and maintain documentation while complying with regulations. The role requires active RN license and on-call responsibilities within the assigned territory.

Qualifications

• Minimum of one year of RN experience in acute care; home health desired.
• Strong communication with patients, families, and physicians.
• Calm, patient, compassionate, and team-oriented.
• Knowledge of nursing best practices and sound judgment.
• Ability to follow directions and work within team and regulatory requirements.

Responsibilities

• Oversee patient care from start to end of the episode and coordinate services.
• Update doctor orders in the patient chart and communicate changes.
• Notify physician of significant changes in condition and adjust plan as needed.
• Provide skilled nursing care at home as directed by the physician.
• Perform assessments, develop and adjust the plan of care, and monitor outcomes.
• Administer medications and promote prevention to reduce hospitalizations.
• Document all care, ensure accuracy, and maintain compliance with Medicare guidelines.
• Work with interdisciplinary team and participate in on-call coverage and case conferences.
