The Christ Hospital is seeking a dedicated Case Manager to oversee utilization review, discharge planning, and care coordination for acutely ill patients in Cincinnati, OH.

You will collaborate with physicians, nurses, social workers, and external payers to ensure appropriate level of care, timely treatment, and patient education throughout the hospitalization and discharge process.

The role requires 3–5 years of medical/surgical nursing and RN licensure, with CCM/ACM preferred.

Qualificações

• Experience in case management, utilization review, and discharge planning.
• Knowledge of case management tools and methods.
• Ability to operate PC-based software and databases.
• Understanding of regulatory and accreditation requirements.
• Strong communication, analytical, and time management skills.
• Ability to collaborate across disciplines.

Responsabilidades

• Clinical review of acute bedded patients against medical necessity criteria.
• Communicate with physicians when alternatives to inpatient care are indicated.
• Monitor appropriateness of continued stay and level of care.
• Work with payers to secure authorizations.
• Develop discharge plans and educate patients/families.
• Identify barriers to discharge and coordinate with care teams.
• Document reviews in Midas/EPIC per UR process.
• Participate in hospital committees and collect quality data.
