The Christ Hospital in Cincinnati, OH is seeking an experienced RN-Case Manager to oversee discharge planning, care coordination, and utilization review for acute patients. This role focuses on patient advocacy, timely transitions, and collaboration with physicians, social work, and external agencies to ensure high-quality care.

The successful candidate will hold a BSN, have 3–5 years of medical/surgical nursing with case management experience, and be licensed in Ohio.

Qualificações

• 3-5 years of medical/surgical nursing experience with case management preferred.
• Education: BSN and active Ohio RN license or eligible for license.
• Required: case management, utilization review, discharge planning experience.
• Knowledge of regulatory and accreditation requirements.
• Strong written and verbal communication and analytical skills.

Responsabilidades

• Clinical review of acute bedded patients including inpatient and observation levels.
• Monitor appropriateness of stay, level of care and services.
• Discharge planning assessment, needs identification, and social service referrals.
• Coordinate with physicians, social work, and external agencies for discharge planning.
• Identify barriers to discharge and develop education-based discharge plans.
• Engage in hospital committees and multidisciplinary rounds.
• Track avoidable days and communicate with care teams.
