Job summary

Unifi Healthcare in the Cincinnati area seeks an experienced RN Case Manager to oversee home health patient care plans, coordinate services, and monitor outcomes in collaboration with physicians and the care team. You will document care, lead care coordination, educate families, and ensure compliance with agency policies.

Current RN licensure and at least 3 years clinical leadership are required; strong communication and computer skills are essential.

Qualifications

• Current RN licensure in good standing.
• Minimum 3 years of progressive clinical experience.
• Ability to work independently and manage caseload.
• Excellent writing, organization, and time management skills.
• Proficiency with electronic health records and basic computer skills.
• Willingness to travel to home visits.

Responsibilities

• Review client POC and document in chart.
• Coordinate and implement care plans with physicians and team.
• Engage with patient families and provide education.
• Attend MD appointments and coordinate coverage as needed.
• Maintain communication with DON/ADON and scheduling.
• Ensure compliance with policies and state regulations.
