Job summary

The Christ Home Health in Ohio seeks a Registered Nurse Case Manager to lead assessment and care planning for patients across West Chester, Camp Dennison, Miamiville and surrounding areas. You will coordinate multi-disciplinary care and ensure quality, cost-effective, and billable services in patients' homes.

Qualifications

• Active RN license in the state of employment.
• Minimum of one year RN experience; home health preferred.
• Excellent communication and documentation skills; patient-focused.

Responsibilities

• Oversees overall patient care from start to end of episode.
• Updates orders and notifies physician of changes.
• Conducts OASIS assessments and develops care plans.
• Provides in-home skilled nursing and administers meds.
