Ohio Living is seeking an experienced RN Case Manager for Home Health to plan, coordinate and deliver quality in-home nursing care. The role focuses on assessment, care planning and collaboration with a broader health team to meet patient needs.

The position requires Ohio RN licensure, at least one year of acute care/medical-surgical nursing experience, and a comfort level with EMR and MS Office tools. It involves on-site home health visits and adherence to regulatory standards.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
• One year experience in acute care or medical-surgical nursing required.
• Experience in a Medicare-certified home health agency preferred.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
• Proficiency with electronic medical records (EMR) preferred.

Responsibilities

• Provides ongoing assessment and nursing care to patients in homes and health care facilities; develops and implements a comprehensive care plan.
• Follows current nursing standards of practice and agency protocols for home health.
• Provides quality plan and symptom management for patient comfort and safety; communicates changes in condition to the care team.
• Oversees the implementation of the patient’s plan of care and evaluates its effectiveness.
• Ensures documentation aligns with policies and regulatory requirements.
