Job summary

Ohio Living is seeking an RN Case Manager for Home Health to plan, coordinate, and deliver high‑quality home health services. You will assess patients, formulate care plans, and collaborate with the care team to meet regulatory standards.

The role requires an active Ohio RN license, at least one year of acute care or med-surg experience, and proficiency with Windows and EMR systems. Evening/weekend coverage and driving to patient homes are expected.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
• One year experience in acute care or medical-surgical nursing required.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
• Proficiency with electronic medical records (EMR) preferred.

Responsibilities

• Care management and service delivery: provides ongoing assessment and nursing care to patients; develops and implements a comprehensive care plan.
• Maintains accurate records and communicates changes in condition to the care team.
• Ensures compliance with federal, state, and local guidelines; participates in quality improvement and audits.
• Other duties as assigned by supervision.
