Job summary

Ohio Living’s RN Case Manager - Home Health position oversees planning, coordination and delivery of quality home health care in accordance with standards and regulations. You will assess patient needs, develop care plans, coordinate services, and communicate with the care team while ensuring compliance and quality metrics across our Ohio Living Home Health network.

Qualifications

Current unencumbered Ohio RN license.
Minimum 1 year acute care or medical-surgical nursing experience.
Medicare-certified home health preferred.
Proficiency with Windows and MS Office; EMR experience preferred.
Must read, write, speak and understand English; valid driver's license.

Responsibilities

Care management and service delivery in patients' homes and facilities.
Develop and implement comprehensive care plans addressing patient needs.
Identify when additional services are needed and coordinate with team.
Maintain compliance with nursing standards and agency protocols.
Communicate changes in condition to the care team; educate patients and families.
Oversee plan of care and evaluate effectiveness on an ongoing basis.
