Ohio Living is seeking a dedicated RN Case Manager for Home Health to plan, coordinate and deliver high-quality in-home nursing care across Ohio. You will assess patient needs, develop comprehensive care plans, and collaborate with health professionals to ensure compliance with standards and regulations.

The role emphasizes quality, documentation, and ongoing education for patients and families, with a mix of field visits and care coordination duties expected to support the home health program.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
• One year experience in a Medicare-certified home health agency required.
• OASIS experience required.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
• Proficiency with electronic medical records (EMR) preferred.

Responsibilities

• Care Management and Service Delivery - Provides ongoing assessment and nursing care to patient in homes and health care facilities. Develops and implements a comprehensive care plan that addresses current needs; identifies when patients need additional services and notifies appropriate team member or leader.
• Follows current nursing professional standards of practice and agency protocols for home health.
• Provides quality plan and symptom management for patient comfort.
• Communicates with other health care providers about changes in condition and patient needs.
• Assesses patient and caregiver needs and provides education when necessary.
• Oversees the implementation of the patient’s plan of care and evaluates its effectiveness.
• Quality and Compliance Management - Audits and maintains current and accurate patient records; participates in quality improvement projects; maintains knowledge of regulatory requirements.
