You will collaborate with families and caregivers under physician orders, ensuring compliance with Ohio Home Health licensure and Medicare standards.

The role requires a BSN or equivalent, an active Ohio RN license, and at least 1 year of direct home health experience.

Qualifications

• Bachelor’s degree in nursing preferred.
• Valid Ohio RN license.
• Minimum of 1 year direct patient care experience in home care.

Responsibilities

• Manage a primary caseload and provide skilled nursing care as prescribed by the physician.
• Ensure care aligns with Home Health Medicare Conditions of Participation and agency policies.
