Qualifications

• Strong communication and patient-family interaction skills; ability to document accurately.

Responsibilities

• Oversee overall patient care from start to finish of episode.
• Update doctor orders in patient chart as needed.
• Notify physician of significant changes in patient condition.
• Coordinate multi-disciplinary care and supervise nursing staff.
• Administer medications as prescribed by the physician.
• Ensure documentation complies with Medicare and agency guidelines.
